Are you eligible?

Eligible borrowers must:

• Be a graduate student 18 or older (depending on your state of residence) enrolled or previously enrolled in a Chase Health Education Program-participating school at least half time
• Attend a participating school—funds can be used for a previous school balance within the last 12 months
• Be a U.S. citizen or permanent resident (or your cosigner must be)

Do you have what you need?

Please have the following information ready before you apply online:

• Social Security Number
• Date of birth
• Permanent address and number of years at that address
• Current address, if different from permanent
• Home telephone number
• Business telephone number
• Reference: Nearest relative or friend not living with you, must be different than reference for cosigner (Name, address, home/business telephone number)
