Please note:
Once you complete all the required information and select your delivery option (e-sign,
download and print, or have Chase mail the documents to you) to receive your application
kit, you will need to complete a self-certification form. The self-certification
form must be signed and returned to Chase along with the Chase Select Private Student
Loan application/promissory note and other requested documentation. If your loan
application is approved, you will receive a Loan Approval Disclosure Form. You must
read and accept the Loan Approval Disclosure Form within the time frame given. Prior
to disbursement, you will also receive a Loan Final Disclosure Form.
